| Our office is based on a fee for service policy. This practice provides only
the highest quality of dental care for our patients. We keep our fees as
reasonable as possible so people can afford our services, however, we will
not compromise the quality of care in order to cut costs and maintain lower
fees. We will cooperate with you in filling out all necessary insurance
forms at no charge to you. You should be aware, however, that we have no
relationship with your insurance company, only with you. You are solely
responsible to our office for services rendered. Your insurance company is
